• Care on a lien: treatment on a signed lien, paid at settlement, with no upfront cost to your client. Billed at our standard fees, the same as every patient.
• Intake confirmation: we let your case manager know when the client has been seen.
• Status updates: a short treatment update to your office every two weeks.
• Records and bills: complete records, narrative and itemized billing within 5 business days of discharge or request.
• Medical-legal quality: Dr. Xavier Ortiz Ramirez is a Qualified Medical Evaluator (QME). Our reports document mechanism of injury, objective findings, and response to care.

How to refer a client

1. Call (909) 300-6479 with the client's name and phone number, or fax a referral to 840-232-4995.
2. Email the letter of representation and claim number to info@lomalindaspine.com.
3. We contact the client, schedule the first visit, and confirm to your office once they are seen.

• Dr. Xavier Ortiz Ramirez, DC, QME: chiropractor and Qualified Medical Evaluator, with a background in emergency medicine and medical-legal evaluation.
• Dr. Juan Pablo Rivera, DC: chiropractor, graduate of Southern California University of Health Sciences, focused on rehabilitation and functional recovery.

Chiropractic evaluation and adjustments, spinal decompression and flexion-distraction, shockwave therapy, red light and laser therapy, rehabilitation exercise, massage and manual therapy, and DOT and occupational medicine physicals.

Hours: Monday and Wednesday 8 to 5, Tuesday and Thursday 10 to 6, Friday 9 to 5, Saturday by appointment.

Treatment is based on medical necessity. Records are released only with the patient's signed authorization. No compensation is offered or accepted for referrals.
